Before anything else on this page: a vent-free gas appliance burns fuel inside the room you are sitting in and puts everything it produces into that room, including carbon monoxide, nitrogen dioxide and water vapor. A working carbon monoxide alarm is a condition of running one, not an accessory. That is set out in full on our page about why a CO alarm is a condition of use, and nothing below changes it.
And the legality question comes first, not last. Whether you may install one of these at all depends on your state, province, county and municipality, and the answer is not the same across a single state. Start with are ventless fireplaces legal where you live and check with your own building department or fire marshal before you spend anything.
This page is about a narrower question. Assume the appliance is legal where you are and correctly installed. Are there households that should still not run one? Yes, and the reasoning is in the codes and in the physics rather than in anyone’s opinion.
The mechanism that decides who is at risk
Everything on this page follows from one fact. There is no flue. The combustion products go into the room, and the only thing removing them is whatever air exchange the building happens to have.
That means the appliance’s output is shared by everyone in the room, and the people who are most affected are the ones whose bodies have the least margin. It also means the risk is not constant: it rises as the room gets smaller, as the house gets tighter, and as the run gets longer. Those are the three variables to hold in mind while reading the rest of this page, and they are covered separately in what carbon monoxide comes out of a vent-free burner.
Households where the answer is no, or close to it
Anyone who would sleep in the room
The model code prohibits fuel-burning appliances in sleeping rooms, subject to narrow exceptions. IRC section G2406.2 states that appliances shall not be located in sleeping rooms, bathrooms, toilet rooms, storage closets or surgical rooms, and the exception for a single wall-mounted unvented room heater in a bedroom is limited to units with an input rating not greater than 10,000 Btu/h, equipped with the safety shutoff of G2445.6, in a bedroom meeting the volume criteria of G2407.5. That exception is not a permission slip. It is smaller than most decorative fireplaces, it requires the room volume to be checked, and whether it applies to your installation is a decision for your local building official. The reason this rule exists is worth saying plainly. Carbon monoxide is colorless and odorless, and a sleeping person gets no warning. CPSC’s newsroom archive carries a release proposing a ban on unvented gas space heaters in 1978, a release approving a mandatory standard in 1980, and a release revoking that standard in 1984. We cite those titles and years only: cpsc.gov returned an HTTP 403 to our request on 28 August 2026, so no figure or quotation from them appears on this page.
Households with someone who has asthma or another respiratory condition
Nitrogen dioxide is a normal product of gas combustion and it is a respiratory irritant. What we will not do is publish an indoor exposure number, because the EPA states that no standards have been agreed for nitrogen oxides in indoor air. The 0.053 ppm figure that circulates in this context is the annual average outdoor limit, applied to the wrong medium. The detail is on nitrogen dioxide and asthma.
Absent a number, the honest guidance is directional and it is still useful: if someone in the household has asthma, COPD, or any condition that a respiratory irritant would worsen, this is a conversation to have with their physician before the appliance is installed, not after the first symptom. A retailer cannot answer that question and neither can we.
Households with someone on supplemental oxygen
This one is not a judgment call. Oxygen equipment and an open flame in the same room is a fire hazard independent of everything else on this page, and the appliance’s own manual and the oxygen supplier’s instructions will both address it. Route this to the oxygen supplier and to a qualified hearth technician before anything is installed.
Households where nobody will be awake and present while it runs
The appliance is designed to be operated while somebody is in the room. A household where the realistic pattern is switching it on and going to bed, or leaving it running in an empty house, is a household whose actual usage does not match the appliance. Your own manufacturer’s manual is the authority on run time, and it is the only authority we will point you at. There is no general figure worth publishing here.
Households in a very small or very tight space
IRC G2445.5 caps the aggregate input rating of all unvented appliances in a room or space at 20 Btu/h per cubic foot of volume, and G2445.3 caps a single unvented room heater at 40,000 Btu/h. Those two numbers together are the code’s way of saying that room volume is a safety input, not a comfort preference.
A tight new build changes the same calculation from the other direction. A modern sealed envelope exchanges less air with the outside, so the same appliance in the same room volume produces a different indoor result depending on how leaky the house is, and a basement compounds it because basements are frequently the tightest and least ventilated space in a home.
Households where the answer is not automatically no, but the margin is thinner
Infants and very young children, and older adults, are groups routinely named in guidance about combustion byproducts in the home. We are not going to attach an exposure threshold to that, because no indoor standard exists to attach. What follows from it practically is that the room-volume and run-time questions matter more in these households, not less, and that the CO alarm is not negotiable.
Pregnancy belongs on the same list and belongs with a physician rather than with a hearth page.
Pets, and birds in particular. Birds are well known to be unusually sensitive to airborne contaminants. If there is a bird in the room, treat that as a reason to choose a different appliance rather than a reason to open a window.
What to do if this page describes your household
There is a real answer and it is not “go without heat.”
If the household is on this list, the appliance category to look at is one where the combustion is sealed off from the room air, or where nothing burns at all. Sealed combustion and direct vent units draw combustion air from outside and exhaust outside, which removes the entire mechanism this page is about, and electric units remove combustion from the question altogether. Where the work stops being yours. Any change to a gas supply, any appliance installation, and any conversion between fuels is licensed work. Call a licensed gas fitter or a qualified hearth technician. This site publishes no gas-line or installation procedure of any kind, at any depth, and it never will.
What this page does not tell you
It does not tell you the appliance is safe for anyone. It gives the code text and the mechanism and stops there.
It publishes no exposure figure, no ppm threshold and no run-time number, because the sources that would license those figures either do not exist (indoor NO2) or belong to your specific unit (the manufacturer’s manual).
And it does not answer the legality question for your address. Only your building department or fire marshal can do that, and our page on asking them the right question exists so that the call takes five minutes instead of three.
